1. Simply print out the file and fill out the Required Document.
2. Sign the bottom of the Document.
3. Fax the Document to 316-689-0662

Once we receive your document, a Vend Tech representative will contact you by telephone to confirm your setup and to formulate any custom product packages and pricing.
